Case Management & Supportive Housing
- Individual Case Management services
provided to residents living independently in the Greater Lowell
Community. Case Managers provide advocacy, support, and
assistance to persons served who are in early recovery, live
independently, and need to access a variety of social services
within the community. These services enable men and women to
improve their coping mechanisms and life management skills, and
continue to build on their foundation of recovery. Case Managers
focus on relapse prevention, daily living skills personalized
after-care planning, housing issues, and vocational and educational
goal attainment. Many of the participants have been involved with
Lowell House, Inc. residential treatment and other LHI programs.
